Home Care Assistance is seeking a Client Care Manager to join our team in Carmel, IN!

The Client Care Manager is responsible for ensuring HCA clients receive excellent care allowing them to remain independent in their homes. The Client Care Manager is also integral to ensuring smooth operations and client satisfaction, supporting long-term retention of cases, and working in partnership with colleagues in operations, staffing, and headquarters support.
Responsibilities of this role:
Speaking to prospective clients to educate them on our home care services, conduct in-home assessments with seniors and/or their families to sign them on for services, perform ongoing care management & communication with our clients for assured high-quality services
Answering calls from referral sources, prospective clients and/or their family members, and existing clients and employees. This requires a 24/7 presence with a high level of accountability and a work cell phone will be provided
Leading orientations, in-services, training, and other company programs or initiatives to enrich the quality of our caregiver staff
Coordinating post-discharge home care with hospital and skilled nursing facility discharge planners, RNs, social workers, and physicians to provide a seamless transition and effective rehabilitation at home for our clients
Answering and actively making calls to prospective clients in need of services
Working closely with the client service team to ensure client satisfaction and excellence of care
Maintaining accurate records on all prospective and active referral sources
Requirements of this role:
Experience with in-home care or elder care as a Business Development professional, Certified Geriatric Care Manager, Client Care Manager, MSW or LVN preferred
5+ years experience in-home care or senior living environment marketing and generating new business from hospitals, skilled nursing facilities, senior living communities and geriatric care managers, etc. preferred
Proven track record of developing and maintaining client accounts
A passion for health and eldercare
Great at building and maintaining relationships
Excellent written and oral communication skills
Superb follow-up skills
Be team-oriented and comfortable collaborating with other staff
Professionalism and the ability to grow our business
