Job Duties
Demonstrates performance consistent with professional standards or practice, care, and performance, and the mission and goals of the organization.
Provides onsite leadership to assure that daily department operations function effectively and efficiently to meet the needs of all customer and regulatory groups.
The Director has empowered this role with the authority necessary to ensure the appropriate clinical outcomes and efficient, effective team process.
Your Qualifications
Registered Nurse or Social Worker
Five (5)+ years of clinical experience in hospital setting
Two (2)+ years of management experience preferred; prefer case management experience, but not required
Certification in Case Management preferred
Registered Nurse:
Graduate of an accredited school of nursing; Bachelor’s degree preferred
Current Registered Nurse license from the State of Indiana
Social Worker:
Graduate of accredited school of social work; Master’s degree in social work
Current Social Work license from the State of Indiana

Demonstrates competency as a Case Manager
Demonstrates team-building skills with accountability to the team
Demonstrates ability to effect change by assuming leadership roles in interdisciplinary teams and supporting the growth and development of team members
Evidence of willingness and ability to assume additional and/or different responsibilities
Demonstrates understanding of quality improvement processes at both the unit and the network level
Understands the relationship of individual patients, units and/or departments to the network as a whole, to facilitate achievement of optimal goals within those relationships
