ProMedica Health System Patient Care Manager - RN - Hospice in Schererville, Indiana

Heartland Hospice a part of HCR ManorCare that provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy is growing and as a result has a full-time Pateint Care Manager position available. This role will involve working in the office as well as providing care to our patients in their homes.

The Patient Care Manager is responsible for the supervision, orientation, instruction and on-going evaluation of all patient care staff who are engaged in the delivery of care. The Patient Care Manager assures compliance with agency policies and procedures, state and federal laws, regulations and accreditation standards.

Minimum of one year experience in home health care, or hospice nursing preferred, may be specified by state.

Previous management or supervisory experience preferred.
